How to get the Mortal Blade in Sekiro
To get the Mortal Blade you’re going to need to head into the Senpou Temple area. This area can be uncovered by heading from Ashina Castle to the Abandoned Dungeon, and then to Senpou Temple/Mt. Kongo.
Once you’ve made it Senpou you will need to progress deep into this region and then enter one of the temples hidden deep in this part of the map. This area is called the “Main Hall” and you will find a Sculptor’s Idol as well as a large statue with many candles surrounding it and a bell. You will ring the bell and then be transported to a boss area where you must complete a puzzle killing four folding screen monkeys.
Completing this puzzle will allow you to access the Inner Sanctum area where you can meet with the Divine Child of Rejuvenation who will give you the Mortal Blade. After receiving the Mortal Blade from the Child of Rejuvenation you can then talk to them again to receive a healing item: Rice.
